I’m really happy that we got to see Liv’s response to the do I or don’t I donate blood situation because we have enough cliffhangers in that episode first of all and it gives us a different sort of cliff hanger, which is a lead in to what will be a much more interesting and emotional story line.

Because if her brother dies, we’ll see a definite tension between her and her mother because it’s unlikely Liv will explain why she won’t give blood to her (or at least not a explanation that won’t lead to more questions and issues), Liv will feel guilty, Liv will have lost another person, and Major will more than likely be torn because he’s the reason she said no and hey the brother isn’t a zombie but also he basically convinced Liv to let her brother die.

Even if the brother does live (which is highly unlikely but possible) a lot of these same issues will come to surface and the brother will also be wondering why his sister was ready to let him die.
